Community Health Assessment survey seeks input
FLORENCE, Wis. — The Florence County Community Health Assessment Survey would like input from county residents.
The assessment is completed every five years as part of the data collection for the CHA and directs the strategies on the Community Health Improvement Plan. The CHIP has a working coalition called the AODA & Mental Health Coalition which drives many prevention and treatment strategies in the community. This coalition has received grant funding from the state consistently for 10 years to collectively address AODA and Mental Health strategies. The survey is one of several ways to allow all eight townships to participate and to intentionally include vulnerable populations through focus groups and one-on-one interviews. Hard copies of the survey are also available through either in-person at the health department or by calling Alisha Krenzke and a self-addressed envelope will be sent to you.
All surveys are anonymous and five, $50 VISA gift certificates are being randomly drawn for participating. Individuals must work or live in Florence County (those who have a vacation home in Florence County are still welcome to participate). A QR code is also available for online completion of the survey.
